Sign Up for COVID Testing at Burley This link is ONLY for students currently registered at Burley. https://home.color.com/covid/sign-up/start?partner=tf-bur-609820-students | Regular testing is a safe, effective way to help prevent the spread of COVID-19 and help keep our schools open for in-person learning. This is especially important for children under 12 years old who are not yet able to get vaccinated against COVID-19, as well as for families and staff with younger children at home. Regular testing will help us keep students in the classroom and allow students to take part in the other activities they love. COVID-19 tests are optional (except for those playing school sports), although strongly encouraged, especially for those who are unvaccinated. Students will not be tested without both the student and guardian giving their consent. COVID-19 tests are required to take part in school sports. Student athletes must get tested once per week in the CPS testing program to be eligible to play in upcoming games and to be on the team. Here are answers to some frequently asked questions: What type of test is it? It is a PCR test, not a rapid test, involving a gentle nasal swab of the inner part of the lower nostril. It is not painful. The company will conduct pooled testing. Material from up to 5 sample swabs will be combined, and each individual sample will be saved in case there is a positive result in that pool. If a pool of tests has a positive result, then each sample from that pool will be tested individually to identify the positive case(s). When do I get results? For each student who is tested, individual results will be provided to parents via email 24-48 hours after the test. Are all students tested? Signing up for testing is optional but strongly encouraged. Students are only tested with parent permission and student consent. The student can refuse on the day of testing. Testing is required for students participating in school sports. Will this disrupt my child’s learning? Testing is quick and interruptions are minimal. The entire testing process should take approximately five minutes. The test is self administered under the supervision of a healthcare professional who can assist the child as needed. How do I sign up? Each CPS school has a unique sign up link. Please do not share this link with people from other schools or use it for children that do not attend Burley. You’ll need your child’s ID number, which can be found on every report card, progress report, or official paperwork that is sent home. You can also find the student ID number on Parent Portal under the student information section. When registering for COVID testing, you will also enter parent contact information and the child’s birthdate, gender, and race/ethnicity. Where can I learn more? As parents, you play an essential role in helping your child succeed. Curriculum Night is an important opportunity to learn about Burley's unique instructional program and the expectations and goals in each grade. We hope all families will watch the videos below and join us on Thursday evening between 5:30-7:00!
To allow for distancing, each classroom will offer 3 identical Curriculum Night sessions. Please plan to attend a presentation by each child's teacher. Choose the time(s) that work best for your family:
From 6:00-7:00, you can visit drop-in Q&A sessions with specials teachers and our school counselor. FIRST: REVIEW DOCUMENTS SECOND: PAY SCHOOL FEE Pay the School Year 2021/22 $125 per student school fee using the Illinois ePay Page for Burley. In the Epay system, choose to make a 1 time payment, fill out the student information and click on the student fee bar. Then click the drop down to pay the $125 student fee option. (student ID number can be found in the student's Aspen account, on report cards, progress reports, etc. If you can't find the student ID number reach out to Celi Aviles [email protected] or call 773-534-5475). If you have multiple children that attend Burley you can build a "cart" and check out all at once in the end for one transaction. Note: We are continuing to utilize these school fee funds to supplement staffing costs, staff professional development, instructional supplies and materials, furniture, equipment and contracts (health classes, copiers, etc) and more. We rely on these funds and appreciate your effort to pay the fee. If financially you are unable to pay at this time in full or have questions about student fees, please email Ms. Aviles at [email protected] so we can get back to you about your individual needs. THIRD: FAMILY INFORMATION FORM Complete the 3 question family information form. This will help inform Burley’s main office of the information that you would like to share with our parent organizations. Click HERE to complete the form. FOURTH: DOCUSIGN REGISTRATION PACKET (You received one email per child) The emails come from “BurleySchool via Docusign” and the subject is “CPS - Please Sign These Back to School Documents for…” This email will only go to 1st contact in Aspen because it can only be signed once per student. (if parents prefer the 1st contact in Aspen to be changed to a different parent or email address please contact Celi Aviles [email protected] or call 773-534-5475). Burley is part of a small group of schools that is completing all emergency forms, family income forms, media release forms, etc. online using a platform that allows parents to digitally sign these materials. These are essential and required forms that must be completed EACH YEAR for all students in CPS. Please complete these documents immediately because there is a compliance due date coming quickly for each document per child. Important Docusign Tips!
